Metal Fabricating Machine Injury

A worker was injured on a metal fabricating machine in New York State. The machine he was working on started to move when it was supposed to be in the "off" mode. The machine and its controls had been recently revamped by a subcontractor. This consultant inspected the machine and found that the rewiring of the controls had left a spurious circuit that allowed the machine to start when it should have been off.
