A trucker was delivering a grain product to a grain facility in central Pennsylvania. The delivery was to a storage bin at the facility, and the trucker had to back into the bin and raise the trailer of his rig to dump the load.
Directly over the dump area were the utility company's overhead bare copper service conductors, consisting of 3 phase conductors plus a neutral conductor, all of which served the grain facility.
As the trailer raised, it contacted and broke the neutral conductor. The broken neutral conductor whipped up and made contact with a phase conductor, which completed a conductive path of current from the phase conductor, through the bed of the trailer, through an employee of the facility, to ground.
The overhead utility conductors had been broken in previous situations and there were no warning signs indicating the presence of the overhead lines.
The utility company was sued for failure to re-route the lines underground, provide warning signs, and provide qualified personnel to avoid the incident. The trucker, who had delivered grain to the location on several previous occasions, was sued for negligence. The suits were settled in court.
