In December 2006, an employee of a steel company was involved in a single vehicle truck accident. The driver was carrying a load of 17 steel beams used in building construction. As the driver was rounding a curve, he lost control and flipped the vehicle, resulting in his death. This consultant was called upon to perform an accident investigation at the scene.
 
After reviewing the scene and interviewing several employees of the steel company, the events leading up to the accident were determined. The driver, a long-term driver with years of experience, advised another driver the day prior to this accident "this curve could kill you."
 
The lack of skid marks in certain areas of the roadway and damage to the surrounding landscaping provided clues to the accident. It was determined that the tractor remained on the roadway and the trailer ran off the road into a ditch. The driver applied power to attempt to maintain control of the vehicle. The trailer’s tires impacted a culvert pipe and caused a reaction that flipped the truck counter-clockwise. The driver was partially thrown from the vehicle and trapped under the vehicle. He died at the scene.
 
The family of the driver received the appropriate Workman’s Compensation. After reviewing the report, the family decided not to pursue further action.
