Mechanical heart valves are durable but recipients require chronic anticoagulation and suffer risks of thromboembolism, hemorrhage, and hemolysis. Bioprosthetic tissue valves normally require no patient anticoagulation but often fail due to calcification and tissue tearing. Despite decades of effort, the biomedical industry has failed to develop a clinical prosthetic polymer heart valve having greater durability than bioprosthetic valves and requiring less anticoagulation than mechanical valves. We evaluated several potential polymer valve materials, as well as surface and bulk modifications designed to inhibit valve thrombosis and mineralization.
SUMMARY AND CONCLUSIONS
Surface modification of both phos-PEU and HP-SR valve leaflets with either fatty acid or hyaluronic acid derivatives prevents both leaflet thrombus accumulation and mineralization of adherent thrombus. However, PEU materials are more susceptible to UV radiation damage during modification than are SR-based materials. Because of design flaws inherent in the valve platform used preclude its clinical use, development of a design more appropriate for these materials is in progress.
